I hereby give my consent for _____________________ to participate in the St. Mark Basketball League. In as much as insurance is not provided by the League, I acknowledge that accidents are not the responsibility or liability of anyone connected with or associated with St. Mark Lutheran Church, and I assume full responsibility for all accidents resulting from my son’s/daughter’s participation.

The following statement is the code of conduct for the League. Each player must agree to abide by the code of conduct.

Each player and coach is expected to remember that the purpose of the league is to provide enjoyable basketball play and instruction in a Christian atmosphere; competition and winning are secondary. Officials are urged

to call technical fouls for swearing, arguing, challenging the calls of officials, and running up the score against weaker teams. It is the coach’s responsibility to stress the importance of good conduct to his/her players, and to set a good Christian example for his/her players. Breaching the code of conduct by a player, parent, or coach will result in the expulsion form the league.

I agree to abide by the St. Mark code of conduct as stated above.
